01 · MEET AMBER
She’d Been Fighting Her Body for 28 Years
Amber is a 42-year-old marketing consultant in the UK. She’d been in a monthly battle with her body since she was a teenager. More than half her life.
By her late twenties, her premenstrual shifts were wrecking her work & her relationships. A PMDD diagnosis didn’t arrive until her early thirties. Five more years of not knowing what was wrong.
She wasn’t short on effort. She’d tried SSRIs. Birth control. CBT. Elimination diets. By September 2023 she was researching hysterectomies & chemical menopause. Not because she wanted surgery. Because she was out of options.

02 · LIFE BEFORE HMM
Half Her Month Gone
Amber came in with a symptom burden score of 464 & a quality of life score of 29 out of 70. Fourteen days a cycle spent in moderate or severe symptoms. Nearly half her month.
What she was dealing with:

- SSRIs. Sertraline & fluoxetine. Minimal shift.
- Birth control pills. Multiple formulations. Some made things worse.
- Progesterone-only pill. Non-stop bleeding & more anxiety.
- CBT therapy. Helpful for coping. Didn’t touch root cause.
- Elimination diets. Cut dairy & gluten. No change.
- Exercise. Couldn’t keep it up through the flares.
She was seriously weighing GnRH agonists (chemical menopause) or a hysterectomy. Not as a first choice. As a last resort.
Why none of it worked.
Every approach was aimed at suppressing symptoms or coping with them. Nothing had actually looked at what was driving them. She was being helped to manage her life around PMDD. Not helped to change what PMDD was doing to her body.
